Integrated Memory Care
Emory's Integrated Memory Care (IMC) is a nationally-recognized nurse-led primary care model that addresses both the unique challenges that dementia patients and their care partners face. It is the only nurse-led center of its kind, treating each patient's dementia and other chronic conditions using a palliative care framework and engaging care partners as a key member of the care team.
A gift to Integrated Memory Care supports the unique challenges and specific needs for the whole family, not just the person living with dementia. Longer appointments, direct phone access, and caregiver support are all made possible by donations from supporters like you.
